1st Floor Sickroom

• Return after dying or reloading the area and talk to Iosefka to get Iosefka's Blood Vial.

Hunter's Dream

• Collect starting gear from Messengers on the stairs

Central Yharnam

• Talk to Gilbert.
• Encounter Cleric Beast on the Great Bridge to unlock the level-up doll in Hunter's Dream.
  • Defeat the beast to get Sword Hunter Badge.
• Talk to Eileen the Crow and get "Shake Off Cape" gesture.
• Pick up Saw Spear.
• Talk to Viola's Daughter and get Tiny Music Box.
• Get Hunter Set in the sewers.
• Get Saw Hunter Badge in the sewers.

Tomb of Oedon

• Defeat Father Gascoigne.
• Pick up Blood Gem Workshop Tool.
• Pick up Red Jeweled Brooch.
• Go trough to Cathedral Ward and talk to Oedon Chapel Dweller to enable sending people to the chapel.

Central Yharnam revisited

• Talk to Iosefka to enable sending people to Iosefka's Clinic.
• Talk to Gilbert to get Flame Sprayer.
• Talk to Lonely Old Woman and send her to Oedon Chapel.
• Talk to Viola's Daughter and send her to the Oedon Chapel.
  • Reload the area and kill the Goliath Pig in the sewers to get Red Messenger Ribbon.

Cathedral Ward

• Talk to Oedon Chapel Dweller after sending at least one person here to get "Triumph" gesture.
• Pick up Hunter Set.
• Pick up Woodden Shield.
• Talk to Alfred and agree to co-operate to get "Pray" gesture.
• Pick up Monocular.
• Purchase Hunter Chief Emblem (10 000 echoes) to open the plaza gate at the top of the large stairs. The area can also be accessed for free after defeating Blood-Starved Beast in Old Yharnam, via a shortcut from Healing Church Workshop.

Old Yharnam

• Leave Djura (the tower gunner) alive in order to get "Brush Off Dust" gesture later on.
• Pick up Rifle Spear.
• Pick up Charred Hunter Set.
• Pick up Bloody Messenger Head Bandage.
• Defeat Blood-Starved Beast and receive Pthumeru Chalice. This will also unlock the door to Healing Church Workshop back at Oedon Chapel.

Healing Church Workshop

• Pick up Formless Oedon (+4) rune and Messenger Urn Festival behind the secret passage in the elevator shaft.
• Pick up Communion (+1) rune.
• Pick up Radiant Sword Hunter Badge at the top.
• Access Abandoned Old Workshop from the tower rafters.
  • Pick up Doll Set.
  • Pick up Old Hunter Bone.
  • Pick up One Third of Umbilical Cord.
  • Pick up Small Hair Ornament.
  • Give the ornament to the Plain Doll in Hunter's Dream to receive Tear Stone.
• Pick up Messenger Top Hat in the rafters of the tower.
• Kill Beast-possessed Soul at the bottom of the tower to receive Beast (+20) rune.
• Open the shortcut door back to Cathedral Ward.
  • Pick up Shabby Set just outside the door.

Cathedral Ward revisited

• Talk to Eileen the Crow outside the chapel to receive "Shhh!" gesture.
  • Head back to Tomb of Oedon and assist Eileen in defeating Henryk to get Heir rune (tier 1), and talk to her afterwards to receive "Approval" gesture and continue her questline. Remember to avoid damaging her as much as possible and keep Henryk from overwhelming her.
• Pick up Black Messenger Hat.
• Talk to Arianna and direct her to the Oedon Chapel.
  • Tell her neightbor, the Skeptical Man, to go to Iosefka's Clinic to make him go to the Oedon Chapel.
  • Talk to her in the chapel to receive "Curtsy" gesture and Blood of Arianna.
• Pick up the Black Church Set
• Defeat Vicar Amelia to get Gold Pendant and the password for the Forbidden Woods (from the altar).

Hemwick Charnel Lane

• Pick up Lake rune.
• Defeat Witch of Hemwick.
• Pick up Rune Workshop Tool.

Hypogean Gaol

• To get here you must be killed by a Snatcher.
• Talk to Adella the Nun while wearing at least the chest piece of any church attire.
  • Send her to the Oedon Chapel and talk to her there to receive "Church Bow (Female)" gesture and Blood of Adella. Remember that accepting Blood of Arianna more than twice after sending Adella to the chapel will make her kill Arianna.
• Pick up Moon rune (10%).
• Pick up Yahar'gul Set.
• Pick up Tonitrus.
• Encounter Darkbeast Paarl.
  • Enter Old Yharnam from the back (Church of the Good Chalice or Graveyard of the Darkbeast lamps) to talk to now friendly Djura. Choose to spare the beasts to receive "Brush Off Dust" gesture along with Powder Keg Hunter Badge.
  • Defeat the beast to receive Spark Hunter Badge.

Forbidden Woods

• Talk to the man in the red lantern shack to receive Tonsil Stone, granting access to the first floor of the Lecture Building.
• Pick up White Church Set.
• Pick up Beast Roar.
• Talk to Suspicious Beggar and send him to Iosefka's Clinic to get Beast rune (+50) later, or kill him in combat here to get it immediately.
• Pick up Cannon.
• Talk to Valtr and join The League to receive Impurity rune.
  • [ONLINE] Successfully co-op with other players with the Impurity rune equipped to gain Vermin. Crush at least 1 and talk to Valtr again to receive League Cane and "League Oath" gesture. Talk to Valtr after crushing 5 and then reload the area to find Master's Iron Helm as well as a hostile hunter that drops Madara's Whistle.
    [OFFLINE] Alternatively, you can also kill Valtr to get One-Eyed Iron Helm and summon the hostile hunter carrying Madara's Whistle.
• Pick up Clear Deep Sea (+100) rune.
• Pick up Deep Sea (+200) rune.
• Pick up Graveguard Set (without mask).
• Pick up Graveguard Mask.
• Pick up Anti-Clockwise Metamorphosis (+10%) rune.
• Pick up Clockwise Metamorphosis (+5%) rune.
• Pick up Dissipating Lake (+5%) rune.
• Defeat the Shadow of Yharnam to receive Blood Rapture (+200) rune.

Iosefka's Clinic

• Pick up Cainhurst Summons.
• If you sent the Suspicious Beggar here, kill one of the Celestial Mobs to get Beast (+50) rune.
• Pick up Communion (+3) rune.
• Defeat Iosefka to get Oedon Writhe (+2) rune, or leave her alive to get One Third of Umbilical Cord later.

Lecture Building (1st floor)

• Follow the instructions on Tonsil Stone to get here.
• Pick up Student Set (vairant A) and Lecture Theatre Key.
• Obtain the Augur of Ebrietas

Nightmare Frontier

• Pick up Fading Lake (+5%) rune.
• Pick up Clockwise Metamorphosis (+10%) rune.
• Pick up Clear Deep Sea (+200) rune.
• Pick up Stunning Deep Sea (+200) rune.
• Pick up Messenger's Gift.
• Pick up Great Deep Sea (+100) rune.
• Defeat Amygdala and receive Ailing Loran Chalice

Castle Cainhurst

• Pick up Reiterpallasch.
• Pick up Noble Dress.
• Pick up Executioner Set (without helmet).
• Pick up Evelyn.
• Pick up Executioner's Gloves.
• Pick up Knight's Set (without helmet).
• Pick up Knight's Wig.
• Defeat Martyr Logarius to receive Crown of Illusions.
  • Pick up Unopened Summons.
  • Kneel before Annelise and join the Vilebloods to receive Corruption rune, Cainhurst Badge and "Respect" gesture.
  • Kill a hunter (NPC or another player) with Corruption rune equipped to gain a Blood Dreg to exchange for "Deep Respect" gesture from Annelise.
  • Bring Unopened Summons to Alfred in Cathedral Ward (near Frobidden Woods entrance) to receive Wheel Hunter Badge and "Church Bow (male)" gesture. Return to receive "Roar" gesture from him and pick up Queenly Flesh. Finish his questline and acquire the Radiance rune by either killing him or going back to find his body in Cathedral Ward (original location behind the mausoleum).

Byrgenwerth

• Pick up Arcane Lake (+7%) rune.
• Pick up Pearl Slug.
• Defeat Hostile Choir Hunter.
• Pick up Student Set (A).
• Pick up Lunarium Key.
• Pick up Empty Phantasm Shell.
• Talk to Master Willem to gain 2 Insight and kill him to receive Eye (+70) rune.
• Defeat Rom, The Vacuous Spider.

Central Yharnam & Cathedral Ward (blood moon)

• If Iosefka is still alive, return to her clinic and kill her to receive One Third of Umbilical Cord.
• Defeat the Old Yharnam Beast at the Central Yharnam lamp to receive Clawmark (+20%) rune and finish Gilbert's questline.
• Kill Adella the Nun at the Oedon Chapel to receive Oedon Writhe (+1) rune.
• If you previously helped Eileen with Henryk, talk to her at the entrance of Grand Cathedral to get "Wait" gesture, then defeat Bloody Crow of Cainhurst to receive Blood Rapture (+300) rune. Talk to her afterwards to receive Crow Hunter Badge and Hunter rune, finishing her questline.
• Talk to Viola's Older Daughter and turn in the Red Messenger Ribbon
• Reload the area and go down the ladder from the window to find the White Messenger Ribbon.

Yahar'gul, Unseen Village

• Pick up Iron Yahar'Gul Helm.
• Pick up Heir (tier 2) rune.
• Pick up Upper Cathedral Key.
• Pick up Tiny Tonitrus.
• Defeat Yahar'Gul Hunters to receive Clawmark (+10%) rune.
• Pick up Arcane Lake (+5%) rune.
• Defeat One Reborn.
• Inspect the mummy to go to Lecture Building 2nd Floor.

Upper Cathedral Ward

• Pick up Great Lake (+4%) rune.
• Pick up Choir Set (without cap).
• Pick up Cosmic Eye Watcher Badge.
• Pick up Blindfold Cap.
• Get the Orphanage Key.
• Obtain Make Contact gesture.

Orphanage

• Defeat Celestial Emissary to receive Communion (+4) rune gain access to Lumenflower Gardens.
• Pick up A Call Beyond.
• Defeat Ebrietas, Daughter of Cosmos to receive Great Isz Chalice.
• If you have Queenly Flesh, place it in the altar to have Annalise available in her chambers again.

Lecture Building 2nd Floor

• Pick up Communion (+2) rune.
• Open shortcut to Lecture Building 1st Floor and talk to Patches the Spider to receive "Beg for life" gesture and Anti-Clockwise Metamorphosis (+15%) rune.
• Exit the area to access Nightmare of Mensis

Nightmare of Mensis

• Pick up Eye (+50) rune.
• Defeat Micolash, Host of the Nightmare to receive Mensis Cage.
• After defeating Micolash, if Arianna is still alive, return to Oedon Chapel and follow the trail of blood from her chair to find her. Kill the Celestial Larva to get One Third Umbillical Cord and Arianna's Shoes.
• Pick up Moon (+20%) rune.
• Pull lever and drop the Brain of Mensis
• Pick up Choir Bell.
• Pick up Blood Rock.
• Obtain Moon (+30%) rune from Brain of Mensis.
• Kill Brain of Mensis and obtain the Living String.
• Pick up Iosefka's Blood Vial.
• Defeat Mergo's Wet Nurse and receive One Third of Umbilical Cord.

Access The Old Hunters (DLC)

• Obtain Eye of a Blood Drunk Hunter from the Hunter's Dream
• Get grabbed by the Amygdala outside the Cathedral Ward lamp

Hunter's Nightmare (DLC)

• Pick up Old Hunter Gloves.
• Pick up Old Hunter Cap.
• Pick up Beast Cutter.
• Pick up Old Hunter Garb.
• Defeat the Beast Claw hunter to receive Firing Hammer Badge.
• Pick up Old Hunter Top Hat.
• Pick up Boom Hammer.
• Defeat the Gatling Gun hunter to receive Gatling Gun.
• Pick up Amygdalan Arm.
• Talk to Simon the Harrowed and answer "Nightmares are fascinating" to start his questline.
• Pick up Constable's Gloves.
• Pick up Constable's Trousers.
• Pick up Constable's Garb.
• Pick up Beasthunter Saif.
• Obtain Butcher Set.
• Obtain Whirligig Saw.
• Defeat Ludwig, the Holy Blade to receive Guidance (+20%) rune.
• Talk to Ludwig's head while wearing any church garb and answer "Yes", or kill the head, to receive Holy Moonlight Sword.
• Pick up Fist of Gratia.
• Pick up Blood Stone Chunk.
• Pick up Laurence's Skull and Church Cannon from the altar.

Research Hall (DLC)

• Pick up Decorative Old Hunter Set.
• Use the hidden path in the elevator shaft to access an Enlarged Head and kill it to pick up Brain Fluid.
• Rotate the stairs on the top floor.
• Kill another Enlarged Head at the very top of the rafters to pick up another Brain Fluid.
• Pick up Enlarged Head.
• Bring the Brain Fluid to Adeline.
  • Give her the first Brain Fluid to get Blood of Adeline (must not have Blood of Adella or Blood of Arianna in inventory).
  • Reload the area at least three times and give her the second Brain Fluid to get Balcony Key.
  • Reload the area at least three times and get the last Brain Fluid from Adeline's new form. Give it back to her to receive Milkweed rune.
• Pick up Loch Shield.
• Pick up Underground Cell Key.
• Use the Balcony Key to access the garden and pick up Blacksky Eye.
• Defeat Living Failures to receive Astral Clocktower Key.
• Defeat Lady Maria of the Astral Clocktower and pick up Celestial Dial.

Fishing Hamlet (DLC)

• Equip Milkweed Rune if you have it and talk to the ranting villager to receive Accursed Brew.
• Pick up Harrowed Set.
• Talk to Simon to finish his questline and receive Simon's Bowblade and Underground Cell Inner Chamber Key. If Simon isn't here, just pick up the key.
• Defeat Brador on the bridge to receive Bloodied Arm Bands.
• Defeat Brador under the house to receive Beast Hide Garb.
• Defeat Brador in the caves to receive Bloodied Trousers.
• Pick up Rakuyo.
• Pick up Blood Rock.
• Defeat Orphan of Kos to receive Kos Parasite, and kill the shadow to finish the nightmare.

Hunter's Nightmare revisited (DLC)

• Return to Nightmare Grand Cathedral with Laurence's Skull to defeat Laurence, the First Vicar and gain Beast's Embrace rune.
• Underground Cells:
  • Return to the cells with Underground Cell Inner Chamber Key to defeat Brador in the hall and obtain Brador's Testimony.
  • Use Underground Cell Key to defeat the imprisoned hunter and obtain Church Pick.
  • Use Undergorund Cell Key to gain access to Old Hunter Yamamura and kill him to obtain Yamamura Set.
  • Use the Underground Cell Inner Chamber Key to gain access to and kill Brador in his cell and obtain Bloodletter.

End Game - Hunter's Dream

• There are three endings and how you proceed will decide which ending you get. NOTE: Once you trigger the end game boss(es) you can not continue in the open world so finish everything you want to before triggering the end.
  • Ending one - Talk to Gehrman and agree.
  • Ending two - Consume less than 3 One Third of Umbilical Cord, talk to Gehrman and refuse.
  • Ending three - Consume at least 3 One Third of Umbilical Cord, talk to Gehrman and refuse. After defeating him, defeat Moon Presence.
